Trouble importing data into Fuelly

When I try to upload my CSV file into Fuelly it says that I do not have enough fields beginning with row 2. I have "odometer", "gallons", & "fuelup_date" column headings. All the rows below are fully populated with data. What am I doing wrong?

I don't know if you have looked at the import guide: Importing Your Data Into Fuelly

If not, please check there.
